Program Description: Home Health’s Value-Based Purchasing (VBP) Model is the latest of the Impact Act Volume to
Value reforms to affect how we develop and deliver care. By redefining HH quality in terms of Pay-for-Performance (P4P), VBP seeks to improve care efficiency.
As a result, traditional HH Care production and management processes fail to achieve VBP-based quality baselines and the related bonus payment.
When reviewing the HH VBP model, it quickly becomes apparent that the depth and specificity employed to track, define,
and reward qualified care extends well beyond the current level of management in most Home Health agencies. The complexity of VBP contrasts starkly
with both the Star Ratings and HHCAHPS models we know, and it is much more difficult to achieve success. Finally, HH VPB will deliver bonus payments
to the top 10% of HH Providers nationally, and few if any Providers could hope to qualify for the bonus without a major VBP-based quality initiative
in their agencies. This progressive presentation tackles the HH VBP reform from all angles for success to assure your V2V performance produces newly identified outcome levels.

Presentation Info:
TAHC&H Winter Conference 2022 | Dallas | Recorded for On-Demand Access
Presentation Date | February 23, 2022
Program Description: Prior to the pandemic, most PDGM education focused on billing or coding aspects of the reform,
with little attention to operational and care delivery requirements. As a result, many HH clinicians don’t understand exactly how PDGM works; the Value-based Operational changes,
and the legacy HH approaches that compromise clinical and fiscal outcomes. With most Providers just returning to a pre-2020 level of care, many of these operational requirements remain un-addressed.
This program addresses areas that directly affect quality care outcomes under PDGM, from both a front-line delivery and in-office managerial aspect. Learn how to hardwire for
tangible results, driving costs down and fiscal margins up, while you improve Star Ratings with single digit readmissions. Learn PDGM Strategies to address case-Mix,
POC development and in-episode control and wound management thru Case Studies and Outcome statistics.

Program Description: Volume to Value (V2V) reforms have rewired the Home Health model in terms of care
development and delivery since the introduction of PDGM. Along these lines, Value-based Purchasing was installed for Home Health,
connecting payment levels to outcomes for the first time in our industry. A review of the requirements for the VBP bonus reveals
outcomes rarely seen to date in HHCAPS, Readmission, Timeliness, and Clinical Outcomes. In addition, HH Readmissions must be in the
single-digits if a Provider hopes to qualify for the VBP bonus, and most Providers post readmit levels in the mid-teens. Case
studies will outline how to start on the path to single-digit readmissions in your agency.
