Program Description: With the implementation of Hospice Quality Reporting Measures and the CAHPS Hospice Survey,
hospice staff and managers must understand the impact of their daily interactions with patients/families and how they may directly influence public
reporting measures (and future star ratings). This presentation provides a brief overview of these regulatory requirements and engages the participants
in exploring each of the questions and the direct impact that staff interaction may have on HIS and CAHPS Hospice Survey responses. This presentation utilizes
case scenarios and group discussion to demonstrate exceptional customer service behaviors that hospice staff can use in daily practice to improve in these measures.

Kimberly Skakhan is Director of Compliance, Regulatory & Quality at Simione Healthcare
Consultants with more than 30 years of clinical, management and consulting experience in home health and hospice. Kim is nationally recognized and highly respected by
state, federal and industry leaders. She has acted as a liaison to state and federal regulatory agencies and legislators to facilitate collaboration regarding issues
affecting the home care and hospice industry. Kim currently serves as a member of the Regulatory Committee of the National Hospice & Palliative Care Organization (NHPCO)
and the Education Committee for the National Association for Home Care and Hospice. She is also a member of the Connecticut Association for Healthcare at Home Kim is a
certified consultant by the Accreditation Commission for Health Care(ACHC) for home health, hospice and private duty care. An AHCC-accredited home care coding specialist
and certified OASIS reviewer(COS-C), Kim earned her B.S. degree in nursing from Southern Connecticut State University, and her M.S. degree in nursing administration from
the University of Connecticut (UCONN), where she has also served as adjunct faculty for the School of Nursing.

Melynda Lee, a member of the Simione team since 2016, is an expert in home care and hospice sales,

hospice leader in Florida and Georgia for 8 years, and is a former board member and education committee member for the Georgia Hospice and Palliative Organization.

Program Description: The Home Health Proposed Rule 2021 was released June 30, 2020 and while there are not significant
changes to the PDGM model there are some details that need to be reviewed! RAPs will have no payments and will have a payment PENALTY if not filed timely.
There are some changes to the wage indices in multiple states with swings in the applicable rates of no greater than 5%. Medical Review began again on
August 3, 2020, across the nation. What does 2020 hold for your reimbursement?

Program Description: Hospice Election Statement changes became effective October 2020.
This presentation reviews the detailed changes to the Hospice Election Statement that are effective October 1, 2020. In addition to the Election
Statement itself, there will also be an addendum requirement that will include many clinicians needing to fully understand what is allowed and not
allowed under the Medicare Benefit and the regulations to support that decision. This presentation assists in implementation of

and cloud computing has made capturing and compiling large amounts of data much easier. However, despite having large
amounts of data, providers are generally faced with the challenge of not knowing how to leverage the data beyond basic

million in annual revenues, including the largest transaction in the company’s history. Prior to Almost Family, Bruenderman was the Senior Director of Development for ResCare, Inc., a national
human services company with more than $1.6 billion in revenue and operations in 42 states. Bruenderman's strategic approach to developing new markets through selective acquisitions led to $350
million in growth for ResCare and launched the company into the Fortune 1000. Today, he has been involved in the purchase of over 125 health and human service companies. Bruenderman received his

Hospice Forum of State Associations. Sherl has served on multiple national, state and local boards and advisory groups including, but not limited to, the National Association for Home Care & Hospice Board,

Jon Terenzetti is the Vice President of Data & Technology Transformation at CareCentrix. In his role, Jon oversees the team responsible for identifying,
assessing, and developing next-generation data, analytics, and technology assets that assist in the delivery of higher-quality healthcare. Throughout his time at the company, Jon has managed key partnerships and
