Program Description: CMS has updated the surveyor protocols and trainings for hospice under direct pressure from the OIG. These surveys are intended on insuring all hospices are providing the
quality of care that we all want in our industry- but it still may increase the difficulty we find with the survey process to ensure full compliance. These rules are being
applied whether you are surveyed by your state, or by an accrediting organization such as Joint Commission, CHAP or ACHC. This session reviews the changes, but even more
importantly provides insights on how to best prepare for these new interview questions, home visit observations and high risk record review areas. Bonus: Tools will be

About the Presenter(s): Annette Lee Annette is a registered nurse, with a Master’s in Health Care Administration, practicing since 1990. For over a decade she worked with the CMS Home Health and Hospice intermediary, Cahaba GBA and CGS, where she provided review of, and education on home health and hospice documentation. She worked as a specialist in the Medical Review, Appeals, and Provider Education departments, and assisted with cross-MAC quality assurance and training.
Annette then founded Provider Insights, Inc. providing consultative support and provider education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers across the nation with Survey Preparation, ADRs and appeals. Today she continues to marry together her experience in the “real world” and the inside knowledge of Medicare to ensure providers can meet the requirements of CMS, and ensure compliant, efficient operations.

Regional Director for Kindred Hospice and Heart to Heart Hospice agencies across Texas. In this role she oversaw an inpatient hospice facility, created and implemented a
companywide QAPI program for an organization that served more than 2,500 patients, and used her experience to create operations that improved compliance and maximized
reimbursements. Zaundra is an active member of the Texas Association of Home Care and Hospice and has served on multiple committees including the Education, Hospice and Medicare/Medicaid committees.

Program Description: This session will outline and detail specific methods, backed up by regulatory references
that can ensure your hospice bills accurately for the care you plan and provide. The emphasis will be on how to ensure that the clinical activities including
eligibility criteria and on-going documentation requirements such as the face to face encounters, the physician narratives, and necessary orders for changes
in condition are included in the billing audit procedures. The speaker will also include information you need to know about transfers to and from your hospice,
billing for contracted physicians, inpatient and respite stays and conditions for payment. This discussion will include principled practices that must be included
in a hospice’s billing policies and procedures in order to mitigate possible implications of fraud and abuse.

Melinda, with more than 28 years in home care, has over 18 years of executive speaking and educating experience,

meetings throughout the country. Melinda has profound experience in Medicare PDGM training, billing, collections, case-mix calculations, chart reviews and due diligence. UPIC, RA, ADR

Program Description: In this session, the four levels of care will be examined to include how the interdisciplinary team determines
the appropriate level of care for each patient. This discussion will include detailed descriptions of each of the hospice team members and how each member influences the quality
of hospice care and ultimately the appropriate level of care. There will be a thorough discussion on how to meet the Interdisciplinary Group Conditions of Participation and how
effective engagement of the team members leads to improved patient and family satisfaction and ethical practices. This session will explore ideas on how to handle situations when
the team doesn’t agree on the appropriate level of care, and how to deal with live discharges incorporating licensure and Conditions of Participation compliance requirements for
these important, potentially life-changing decisions.

About the Presenter:
J. Robert Parkey is a sixth-generation native of the Wichita Falls area, and
received his Doctorate in Medicine from the University of Texas Medical Branch in Galveston, and then completed his residency in Family Medicine
at the University of Oklahoma campus in Tulsa, where he served as Chief Resident. In 2010, he completed a Masters degree in Theological Studies
from the University of Dallas in Irving, with a personal emphasis in Biomedical Ethics.
Dr. Parkey practiced rural medicine in Henrietta, Texas for 17 years, but found a special love for the care of the elderly,
those with dementia, and for patients with terminal and life-limiting illnesses, expanding his medical specialization and joining the medical staff of
Hospice of Wichita Falls in 2008, then closing his rural practice in 2013. He is a member of numerous medical societies, including the American Academy
of Hospice and Palliative Medicine, the Society for Post-Acute and Long-Term Care, the American Geriatrics Society, the American Society for
Bioethics and Humanities, and the Catholic Medical Association.
His “spare time” is filled caring for his family ranch in Clay County, and trying to keep up with his wife Katie
(a Registered Nurse who helps him run his private practice and who serves in a number of volunteer capacities in the area) and their three
daughters. His hobbies include fishing and literature, and he is a member of the Wichita Falls Poetry Society, the Texas Poetry Society, and
the Academy of American Poets.
Dr. Parkey holds board certifications in Hospice and Palliative Medicine, Family Medicine, and board
certification by the Hospice Medical Director Certification Board. He also holds board certification from the American Board of
Post-Acute and Long-Term Care Medicine.

Program Description: Medicare Administrative Contractors (MACs), Unified Program Integrity Contractors (UPICs) and
Supplemental Medical Review Contractors (SMRCs) are busy in the hospice industry right now. As CMS and contracted agencies improve their use of data to evaluate
outliers in the hospice community, it’s crucial for providers to understand how data is used, where to find provider information, and how to evaluate documentation
in cases in Long Length of Stay (LLOS) cases. This session will cover the basic factors that contribute to LLOS and identify the proper tools providers can
utilize for tracking decline. The session will also cover the process of completing documentation reviews, what to consider in determining ongoing decline
and terminality, and how to respond to a request for additional review.

Lisa Meadows has over 25 years of experience in the health care industry; from acute care hospitalization, home healthcare,
to hospice and palliative care. She is currently the Clinical Compliance Educator for Accreditation Commission for Health Care (ACHC) - a nationally-recognized non-profit accreditation
organization that has CMS Deeming Authority for Home Health, Hospice and DMEPOS and am responsible for educating organizations on the ACHC Accreditation process as well as assisting
providers with interpreting the ACHC Standards and Medicare Conditions of Participation.
Lisa also successfully prepared and lead the home health and hospice agency where she was previously employed by through the ACHC Accreditation and Medicare
recertification process. Lisa have also worked as an ACHC Hospice Surveyor, allowing me the opportunity to assist organizations with developing best practices and compliance strategies

Over the last year, CMS has been digging into their proverbial treasure chest of regulations to implement new rules for Hospices.
After a strong push-back from industry associations, the new Hospice Special Focus Program (SFP) has been placed on hold for the time being.
The new HOPE tool is following the HHA OASIS structure to provide new assessment time points and measures based on new data items.
CAHPS Hospice now has fewer questions including a new option for web-based surveys, a mode in use by Medicare Advantage patients.
There is a lot to unpack with all these changes for FY 2025. Join our speakers as they review these planned

including audits and appeals related to the OIG and program integrity contractors as well as compliance risk assessments related to mergers and acquisitions.
She is actively involved in the educational and advocacy activities of NAHC and NHPCO and is a member of the HHFMA Innovation, NAHC Education and Women in

With the OIG elevating hospice eligibility as a top oversight priority and ZPIC/RAC reviews intensifying in Texas, agencies cannot afford inconsistent or informal eligibility decisions. Errors now carry real consequences: recoupments, payment suspension, loss of Medicare certification, and reputational damage.
