CY2024 Home Health Final Rule: VBP

What’s now and what’s new? Presentation Info: TAHC&H Event Administrator Conference | Rich-Media Presentation Capture | Online On-Demand Access Presentation Date | November 8, 2023 Program Description: What’s now and what’s new? The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes. Even though CMS is omitting the TNC from its programs and replacing it with the Discharge Function score, the TNC scores are still important through payment year 2026. Now is the time to ramp up for the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based) quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. PPH will include hospitalizations (both ACH and LTCH) and observation stays. The DTC measure will include readmissions for up to 31 days after discharge. Lisa discusses what actions can be taken now to improve those numbers. [3823] Will your Hospice Documentation Withstand the Scrutiny of a Medical Review Audit?

Presentation Info: TAHC&H Event Administrator Conference | Rich-Media Presentation Capture | Online On-Demand Access Presentation Date | November 8, 2023 Program Description: Accurate hospice eligibility documentation is critical to fulfilling the CoPs and payment requirements. The lack of supporting documentation for a terminal prognosis is the number one reason for denial. Reviewers often look for a significant decline in patient condition. Although this is not a requirement of hospice care, terminal prognosis is. This program helps clinicians document the slightest changes in baseline measures. More importantly, you'll learn how to capture in documentation the occurring changes that support terminal prognosis – even without a decline in baseline measures. Prepare for 2024 with a Financial & Operational Perspective of the Hospice Final Rule

[3822] Prepare for 2024 with a Financial & Operational Perspective of the Hospice Final Rule
This program is a MUST for all Hospice staff. Presentation Info: TAHC&H Event Administrator Conference | Rich-Media Presentation Capture | Online On-Demand Access Presentation Date | November 8, 2023 Program Description: It is crucial for hospice agencies to understand how the Hospice Final Rules impact agency operations both financially and operationally. The Hospice Final Wage Index & Payment Rule is packed full of changes for 2023-2024 including a projection of 3.1% increase in total hospice payments. However, it is not an overall increase in reimbursement rates for everyone due to the significant and widely varying changes in the wage index that drives the local rates an agency receives from Medicare. This program also includes strategic insights with a comprehensive breakdown of the updates to Hospice Conditions of Participation and Hospice Quality Reporting Requirements. See SimiTree’s national speakers, Mike Simione and Laurie Newlun, as they delve into the conversations that matter. You will gain the knowledge and resources for preparing your agency for the impact of both the value piece and financial piece as well as outline anticipated actions providers should take now. This program is a MUST for all Hospice staff. Why and How Value-Based Insurance Design (VBID) Matters to Your Hospice Organization

[3798] Why and How Value-Based Insurance Design (VBID) Matters to Your Hospice Organization
How VBID compnents affect your hospice and revenue collections. Presentation Info: TAHC&H Event 54th Annual Meeting | Rich-Media Presentation Capture | Online On-Demand Access Presentation Date | August 31, 2023 Program Description: In its fifth year of testing, the hospice component of Medicare’s Value-Based Insurance Design (VBID) continues to grow and evolve. In fact, the number of Medicare Advantage organizations participating in the VBID model increased by more than 35% and were expected to provide coverage for more than 9.3 million Medicare beneficiaries in 2023. While those numbers are still out, what we do know is that if your hospice organization has not been affected by this shift in payment for hospice services, it will be in the very near future. In this session, Zaundra Ellis discusses components of the hospice VBID model, how they affect the care your hospice organization provides, and how collecting your hard-earned revenue is different when working with a Medicare Advantage organization.
