Program Description:With an ever-changing competitive market, providers need to consider
how to deliver services more efficiently. This program takes a look at real life case studies of how technology is being implemented
in home and community care to assist in the delivery of services. What technologies are being implemented to record and monitor changes
in an individual’s health? What is being implemented in regional and remote areas to ensure the wellbeing of seniors?
Machine learning is a type of artificial intelligence that is concentrated on developing algorithms that
analyze data and predict future events with tremendous accuracy. It is a technology that is well suited to identify hidden trends and
patterns in vast amounts of data, not visible to the human eye. Jonathan Vallée, CTO of AlayaCare, an end-to-end home healthcare software
solution, discusses the importance of collecting and analyzing data, provide real examples of feasible tech solutions organizations
could implement today, explore the insights gained from utilizing machine learning, and present the results of two studies on applying
machine learning to streams of incoming remote patient monitoring vitals for decision support.
Program Learning Outcome: Participants will actively engage in the learning activity and
indicate an intent to change their practice by implementing remote patient monitoring as a tool to aid in improving optimal patient outcomes.

Program Description: Transitions in care would seem on the surface
to be a "non-issue" in hospice medicine, but with increasing complexity and segmentation of care for the
seriously ill, patients often enter hospice from the acute care system with unrealistic expectations of
what is needed and what will be provided. Often unnecessary emergency department visits and hospitalizations
result, as well as demands for services not part of the standard hospice plan of care. This program
explores not only addressing the expectations of patients and their families, but also addressing the
increasing expectations of payers. A vision for the future of optimal care of the seriously ill is
presented, acknowledging the daunting steps needed for the evolution of a hospice organization towards
a "full service" model, as our organizations enter into a time of increased demand, regulatory uncertainty,
quality-based scrutiny, and fiscal constraint.
Program Learning Outcome:
Participants will actively engage in the learning activity and indicate an intent to change their practice by incorporating
effective communication strategies and comprehensive care plans to avoid unnecessary transitions of care.

§558.260[a][3]basic principles of management in a licensed health-related setting
Program Description: There are many situations that may appear to be unique to home care, but don’t be fooled … employment law does not make
any exceptions for this industry. In addition to a survey of the most significant hiring, policy, and work separation issues, and an update on how the agencies and courts have been
ruling on employment claims and lawsuits, Mr. Simmons offers a detailed look at common wage and hour law problem areas such as:
exempt vs. non-exempt employees
on-call time
travel time
deductions from pay
hourly vs. contract workers
PRN
live-ins
companionship exemption
final pay issues
This program presents the most important compliance issues for payroll and HR in Texas, as well as the most
important and useful best practices for employers to follow in order to minimize the risk of employment claims and lawsuits.
Program Learning Outcome:
Participants will identify how to remain in compliance with wage and hour laws.

Program Description:The failure to timely report A&E to the Department of Family and Protective Services a
nd to DADS remain one of the higher range license violations among home and community care service agencies in Texas. There are several reasons for this.
First, many times management staff have the misunderstanding that they must make a judgment call as to whether or not an allegation has been made.
Second, middle management staff and delivery staff usually fail to understand how to dignify an allegation estimate.
This presentation examines final orders from the Texas Health and Human Services commission dealing with allegations of
abuse, neglect and exploitation. By examining these cases, as opposed to discussing a hypothetical story, the viewer will gain a better
understanding of what goes wrong when an allegation of abuse, neglect and exploitation is not timely reported to the authorities.
Program Learning Outcome:
Participants will actively engage in the discussion and incorporate best practices for identifying and reporting abuse, neglect, and exploitation into their clinical practice.
