Program Description: Prior to Jan. 13, 2018, many of the things we “kinda” did okay, such as care coordination, comprehensive assessment and plan of care management, became mandatory measures under the new Conditions of Participation. And agencies have struggled! Surveyors continue to site quality of care, care coordination and care planning collectively as one of the top two deficiencies for agencies across the board. By having an in depth understanding of Medicare’s definition of the four “C’s” along with how they build upon each other will lead to more streamlined processes, develop effective care coordination practices and provide better, more patient-centered plans of care thereby helping to “survey proof” your agencies.

Program Learning Outcome: Participants will actively engage in the learning activity and indicate an intent to change their practice by developing effective care coordination practices in line with the updated home health conditions of participation to contribute to improved patient outcomes.

About the Presenter:
Apryl Swafford
began her nursing career in 1993, obtaining a BSN from Jacksonville State University in Jacksonville, Alabama. Her first few years were spent learning nursing, trial by fire style, in a 16 bed ICU unit. She transitioned into home health in late 1997 working PRN and weekends while learning her way and has spent the majority of the years since working home health in some capacity. She has worked as a field clinician/case manager, clinical manager and leader of a cardiopulmonary specialty program. During her tenure at the last two positions, she was responsible for chart audits, OASIS review and coding plus management of a team of nurses, therapist and home health aides. Apryl is certified as an OASIS Specialist - Clinical (COS-C), and is a credential holder of the Homecare Coding Specialist (HCS-D) for both ICD-9 and ICD-10. During her free time, she enjoys her family (there’s a lot to enjoy with 4 children and 9 grandchildren), working on the family’s small farm and running.
