Though the American Medical Association and the Joint Commission offer recommendations for discharge planning, there is no universal system used within hospitals. Being able to work with your hospital discharge planners to help them make fact based transition of care decisions, even if it’s not to your practice setting, may influence the rest of patients’ lives. Program Learning Outcome: Learners will actively engage in the learning activity and indicate an intent to change their practice by utilizing techniques to help discharge planners make fact-based decisions when assisting with transitions of care.
