Program Description: Dementia is a progressive disease that affects many of our home health and hospice patients.
Providers who care for dementia patients in the home setting face a unique set of challenges, especially as the disease becomes more advanced.
This program provides an analysis of the types and manifestations of dementia and how it affects the patient, family, and caregivers. It also explore the importance of disease
management and educating caregivers on best practices and strategies to improve the quality of care provided and to allow the patient to remain as independent as possible for as long as possible.

Program Description: Wound care, in general, continues to be an area of concern with most agencies when it comes to
assessment and documentation. Many clinicians and staff that audit charts struggle with the different types of wounds, what needs to be documented, and how to
document findings to paint a picture of what is happening with that wound. Any clinician who performs wound care or any staff member that is responsible to
review wound documentation should view this program. The presenter takes a practical look at wounds and best practices for assessment and documentation of wounds
and talks about the different types of wounds, the required physician documentation, methods and frequency of measurements, and
