This program presents the pros and cons of working with MCOs in and out of network. As
traditional payor sources implement payment reductions, new payment models or move exclusively
to Managed Care, home health agencies are once again looking at the best way to work within the
Managed Care arena. Some MCOs can pick and choose their provider networks based upon
geographical area, STARs reporting, and pricing models. Medicaid MCOs typically offer below
market value contracts. Although the MCO must follow the home health rules, the payer can
establish additional requirements such as prior authorization, timeframes for physician signatures,
etc. that can be more stringent than the traditional counterpart. The avenues for appeals are
cumbersome and often leave the provider “holding the bag.”
Learn how to package your agency for
quality, cost and satisfaction in advance of seeking contracts with the MCOs in your area. Delivering
care in accordance with MCO requirements requires process modifcation for order development,
authorization management and billing. In addition, the presenter touches on best practice processes to improve

Program Description: What’s now and what’s new? The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes.
Even though CMS is omitting the TNC from its programs and replacing it with the Discharge Function score, the TNC scores are still important through payment year 2026. Now is the time to ramp up for
the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your
ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based)
quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. PPH will include hospitalizations (both ACH and LTCH) and observation stays.
The DTC measure will include readmissions for up to 31 days after discharge. Lisa discusses what actions can be taken now to improve those numbers.

Program Description: This program presents the components of the Hospice Quality Reporting Program (HQRP) and the financial impact
for hospice providers who do not meet the reporting requirements. You will learn the changes in the HQRP that were finalized in the Hospice FY 2023 payment rule
including the new Hospice Care Index (claims-based) measure, an update on the Hospice Visits in the Last Days of Life (HVLDL) measure, STAR rating for the
hospice CAHPS survey and removal of the seven HIS measures from Care Compare. This session will also provide an update on the HOPE tool.

About the Presenter(s): Leslie Heagy is Registered Nurse who joined Healthcare Provider Solutions, Inc. (HPS), as the Clinical Services Manager,
in June 2017. Leslie is part of the clinical consulting team providing support and solutions to home health and hospice agencies across the country to achieve regulatory compliance.
In 2022, Leslie was promoted to Director of Clinical Services over both home health and hospice clinical consulting.
Leslie’s commitment to service and compassionate leadership first took root 21 years ago when she began providing care to patients as a hospice nurse after
receiving her nursing degree from Ivy Tech Community College in Columbus, Indiana. Throughout her nursing career, she has worked in various capacities within the home health,
hospice and private duty industries. Her experience includes successfully opening a Medicare/Medicaid licensed hospice start-up program, leading multidisciplinary teams
in various locations to being part of the team to open a new hospice inpatient unit. She has educated hospitals, skilled facilities, assisted living facilities
and the community on the benefits and requirements for home health and hospice services. Leslie’s extensive Federal and State Regulatory knowledge, along
with her vast clinical experience motivates her to continually seek solutions to the barriers of healthcare in all settings.
Leslie has operational experience in the survey process for Medicare/Medicaid licensed agencies and has assisted agencies in developing/implementing
corrective action plans. Her experience includes clinical consulting, performing clinical chart reviews, onsite billing audit reviews, provider education, developing and
implementing quality and performance improvement programs for home health and hospice agencies.
