Regional Director for Kindred Hospice and Heart to Heart Hospice agencies across Texas. In this role she oversaw an inpatient hospice facility, created and implemented a
company wide QAPI program for an organization that served more than 2,500 patients, and used her experience to create operations that improved compliance and maximized reimbursements. Zaundra is an active member of the Texas Association of Home Care and Hospice and has served on multiple committees including the Education, Hospice and Medicare/Medicaid committees.

Program Description: This presentation covers the details of the Value-Based Insurance Design (VBID) Model – Hospice Benefit Component.
Medicare Advantage (MA) has long had coverage for home health patients, but there has never been coverage for Hospice under the MA benefit. VBID is a demonstration that will begin
January 1, 2021. There are 9 Medicare Advantage Organizations (MAO) that will be participating and have coverage in multiple states. In addition to Hospice care there is an expansion
of Palliative Care services that will be offered to patients through the MAO. CMS is testing how the hospice benefit component can improve beneficiary care through greater care coordination,
reduced fragmentation, and transparency in line with recommendations by the Office of Inspector General (OIG), the Medicare Payment Advisory Commission (MedPAC) and others.
CMS will require that MAOs provide beneficiaries with broad access to the complete original Medicare hospice benefit. This presentation covers all known aspects of the demonstration and prepares
hospices for the reality that Medicare Advantage is now in the Hospice world.

Program Description: Major scrutiny of the Hospice Benefit is well underway nationwide. This program reviews the details of both programs and what Hospices can expect.
The Special Focus Program (SFP), will monitor hospices identified as poor performers based on selected quality indicators. Hospices selected for the SFP will be under additional oversight to enable continuous improvement.
This required program includes the development and implementation of enforcement remedies for noncompliant hospice programs, as well as procedures for appealing determinations regarding these remedies.
These enforcement actions can be imposed instead of, or in addition to, termination of the hospice program’s participation from the Medicare program. Through the PPEO, new providers or suppliers are
subject to prepayment medical reviews designed to address improper billing practices right out of the gate. This program is for newly certified hospices as well as change of ownership and a must-view!

managed care organizations (MCOs), states have a unique opportunity to use MCO contracts and relationships to accelerate wide-scale adoption of value-based
payments (VBP). Texas is jumping on board with this concept and your agency needs the tools to succeed in this new world. It is key for providers to
position themselves successfully to contract with MCOs by understanding the data needed to develop mutually agreeable quality based contracts.
During this presentation you will hear directly from HHSC about how they are requiring MCOs to develop alternate payment structures between them
and their health care providers to encourage innovation, quality and efficiency. Hear directly from Jami Snyder, Associate Commissioner, Medicaid CHIP, who

Jami Snyder is the Associate Commissioner for the Medicaid and CHIP Services Department
at the Health and Human Services Commission, and serves as the state's Medicaid director. Jami most recently served as Chief Deputy Director for Medicaid and CHIP for HHSC.
Prior to that, she served as the Chief Operating Officer for The University of Arizona Health Plans (UAHP), a division of Banner Health. She has over 18 years of experience
in both the public and private sectors, including extensive regulatory experience stemming from her tenure with the Arizona Health Care Cost Containment System (AHCCCS) and
the Arizona Department of Health Services, where she was charged with overseeing the state's Medicaid-contracted health plans, including compliance with established federal
and state managed care regulations and performance in a variety of areas such as network sufficiency, payment modernization, customer service and the provision of self-directed
care. Snyder graduated from Gustavus Adolphus College with a Bachelor of Arts in Political Science and has a Master of Arts in Political Science from Arizona State University.
About the Presenter:
Matt Ferrara is the Director of the Office of Quality Oversight within the Quality and
Program Improvement Section at the Texas Health and Human Service Commission (HHSC). This office manages and coordinates quality initiatives within the Texas Medicaid
and CHIP programs, as well as across the various Texas Health and Human Service agencies. He has held this position for over 5 years. Prior to his employment at HHSC, Matt
worked in the mental health and substance use disorder fields for 13 years for the Texas Commission on Alcohol and Drug Abuse, TDMHMR, and Texas Department of State Health
Services. Before state employment, Matt served for 8 years in direct service and executive capacities for a private provider of community based long term service and
supports in Central Texas and El Paso. He is a graduate of Texas State University San Marcos with a major in Social Work.
About the Presenter:
Andy Vasquez has over 20 years of Texas Medicaid experience. During those years he has led
teams through several evolutionary changes in Medicaid pharmacy benefits management. He spent the last 10 years serving as director of the Medicaid Vendor Drug Program and
now serves as a Medicaid & CHIP Services Deputy Associate Commissioner. He leads five offices that recently joined to form the Quality & Program Improvement Section;

Kay Hollers is well known to TAHCH audiences as she has been an active member or associate member since her return to
Texas in 1985. Kay holds a Bachelor of Science degree from the University of Texas with a major in Nursing and a Master’s Degree in Public Health from the University of Texas School of
Public Health. She also earned a post-master’s Specialist Certification in Health Care Evaluation from Wayne State University College of Nursing in Detroit. Kay taught Public Health Nursing at
Michigan State University and has practiced in and consulted with home health and hospice agencies large and small, public and private. She currently is a consultant with HealthCare Consultlink.
Kay likes to share her experiences and knowledge with audiences and has been a frequent presenter for TAHCH conferences and seminars.

disabilities who are aging out of pediatric Medicaid programs. Each person’s rights for personal autonomy, liberty, freedom and dignity is one that is to be respected and supported.
By law, each adult or emancipated minors presumed to be competent to make decisions for him/herself. Many families need guidance and education to navigate this pathway successfully
with their loved one. Knowing what options are available to families for their loved ones is a difficult path to manage, but with the information provided in this session, agency
leaders will know what resources are available and how to guide families to the legal path that is best for them.

Terry Anstee is a staff attorney at Disability Rights Texas, the federally-funded protection and
advocacy organization charged with protecting the rights of persons with disabilities in Texas. In this role, Terry has represented many children in fair hearings on
a variety of Medicaid issues, including Comprehensive Care Program services and Medically Dependent Children Program services. Terry is also a Registered Nurse and
his nursing experience helps him to understand client’s medical issues and clinical records. He has dealt with Health and Human Services, Texas Medicaid and Health
Partnership, and Medicaid Managed Care organizations on a variety of issues and their obligations under the law.
Courtney Carey received her BSW from Tarleton State University and her MS in Long Term Care, Senior Housing and Aging Services
from the University of North Texas. Courtney is also a Texas Certified Guardian. She has worked for Denton County MHMR for almost 18 years with both the Mental Health and
IDD services. Courtney is the Administrator of Provider Services which encompasses the Denton County Guardianship Program and the Home and Community Based Services (HCS)
Medicaid Waiver Program. Both are a provider of last resort for Denton County.
Travis Weaver is an attorney with Travis Firm Attorneys in Rhome, Texas who provides his community with deep legal knowledge
and strong personal commitment to helping obtain nursing home benefits, protect assets through a will, claim ownership of inherited property or cash, and negotiate

About the Presenter(s): Tony Meluzio is the Senior Content Manager for Axxess, the leading technology innovator for healthcare at home.
Tony is responsible for the design and development of the Axxess Training and Certification Program. During his tenure, he has been able to advance Axxess’ educational products all
while decreasing seat time for training while improving outcomes.
Tony comes to Axxess with over 20 years of experience in the healthcare space, with a primary focus on home health. He spent 10 years at the LHC Group beginning his career as a
clinician and then advancing to corporate training where he designed and developed workforce management solutions, decreased onboarding time and turnover, created OASIS training,
clinical specialty programs and survey readiness education. Tony also served as a consultant to other teams in the organization, detailing how to better deliver content and improve
employee engagement. Since 2018, he has served as a professional consultant for EMR system optimization, survey readiness and documentation review.
Tony is a volunteer WVU Extension Master Gardener Program, where he leads community education efforts related to agricultural practices, sustainable living, raised bed gardening
and a seed bank. He is also active in his local rotary chapter and the Pay It Forward Program, where he helps collect and distribute gently used items to people in need.

Tom Knapp is a member of Brown & Fortunato’s Corporate Group, where he is a shareholder. Tom serves as the lead advisor to buyers and sellers in numerous

The Patient Driven Groupings Model (PDGM) is sure to be a game-changer for the way agencies are operated and care is provided in the home. This session will discuss how PDGM is affecting the mergers and
acquisitions market of home health and hospice agencies, what the valuation drivers are and the impact on budget, cash flow, growth and sustainability.

Brian Bruenderman brings almost 20 years of M&A experience to Stoneridge Partners - nearly a decade has been devoted
exclusively to companies serving the health and human services arena.
Brian Bruenderman joined Stoneridge Partners after a successful stint as Director of Business Development for Almost Family, a leading provider of home health nursing,
rehabilitation and personal care services with over 250 locations in 15 states. During his tenure, despite much uncertainty in the home health industry at the time, Almost Family acquired $160
million in annual revenues, including the largest transaction in the company’s history. Prior to Almost Family, Bruenderman was the Senior Director of Development for ResCare, Inc., a national
human services company with more than $1.6 billion in revenue and operations in 42 states. Bruenderman's strategic approach to developing new markets through selective acquisitions led to $350
million in growth for ResCare and launched the company into the Fortune 1000. Today, he has been involved in the purchase of over 125 health and human service companies. Bruenderman received his
Bachelor of Arts degree and Juris Doctor from the University of Louisville. While in college, he purchased several small businesses and credits his later success to that experience. His
business background and legal education give him the skills and perspective to successfully negotiate transactions all the way through closing. Specifically, his almost two decades of involvement
