juncture in its evolution from fee-for-service (FFS) to a healthcare system based on the delivery of value.
This program includes discussion and review of Medicare Home Health claims data and how it supports the arguments
that will be made. The presenter starts with a quick look at the current state of the industry and some of the
new delivery models that are requiring reassessment of how home health is delivered (ACOs, Bundled Payments, etc...)

About the Presenter: Duane Blackwell has been in home health since August, 1997 when he was
hired as CEO of Thompson Home Health, a multi-provider Louisiana-based home health company. At that time, the industry was undergoing a
dramatic transition from a cost-based system to prospective payment. Duane’s previous career in commercial banking seemed a good fit for
what was to be a tectonic shift in the way home health was delivered. In 2003, he assisted in the successful sale of Thompson to an equity
investment firm, staying on as CEO for five years following the sale.
In 2008, Duane established SOSO Solutions, LLC, a consulting practice focused primarily on the serving needs
of post-acute care providers, including home health and hospice companies. SOSO’s areas of focus include strategic planning, financial
management, the management of change, and sales and business development. SOSO continues to serve a client base centered in the Gulf South..

About the Presenters: Sherl Brand is a registered nurse and health care executive with over twenty-five years of experience in
both for profit and non-profit organizations. Currently Sherl is Senior Vice President of Strategic Solutions at CareCentrix focusing on strategy, network engagement and partnerships. Prior to joining CareCentrix,
she held the roles of Chief External Affairs Officer with Visiting Nurse Association Health Group, a state-wide non-profit home health, hospice and community based organization,
National Director of Managed Care Contracting for Bayada Home Health, a national for profit home health, hospice and private duty organization and VP Government Affairs & Business development for
Patient Care, a multi-state for profit home health and private duty nursing organization.
Sherl also served as the CEO of the Home Care & Hospice Association of NJ and its related Home Care Foundation. She was also elected Chair of the National Association for Home Care &
Hospice Forum of State Associations. Sherl has served on multiple national, state and local boards and advisory groups including, but not limited to, the National Association for Home Care & Hospice Board,
National Government Services Provider Advisory Board, Hospital Joint Venture Boards and Partnership for Medicaid Home Based Care. Sherl received her B.S.N. from Ohio Wesleyan University and was a Certified Case Manager from 1993-2008.
Jon Terenzetti is the Vice President of Data & Technology Transformation at CareCentrix. In his role, Jon oversees the team responsible for identifying,
assessing, and developing next-generation data, analytics, and technology assets that assist in the delivery of higher-quality healthcare. Throughout his time at the company, Jon has managed key partnerships and
capability buildouts with machine learning and artificial intelligence partners that help deliver better health outcomes to 25M+ lives under management. Prior to his current role, Jon acted as Chief of Staff to the CEO.
Jon’s career has been focused on patient engagement. Before CareCentrix, Jon spent years working at CVS Health, overseeing a range of initiatives, including the redesign of the
pharmacy ExtraCare rewards program, the launch of CVS’s audiology and optometry services in select markets nationwide, patient segmentation and acquisition efforts, retail technology rollouts, and
numerous gross margin and operational efficiency initiatives. Before CVS, Jon worked at Target Corporation in financial forecasting and planning. Jon holds a BA in political science from Yale University.

Program Description: For the last several years, the failure to timely
report allegations of ANE to the appropriate authorities ranks in the top 10 licensure violations for home health
and hospice providers. For home health (including skilled care and PAS), it has been the third most commonly cited
violation coming in behind management responsibilities of both the supervising nurse and the administrator. Usually
one or both of those is cited along with the failure to timely report. Almost without exception, there are penalties
assessed with each failure to timely report. At times, the failure will result in license revocation. More times
than not the failure to report is due to agency staff misunderstanding this obligation or misunderstanding what
constitutes an allegation of ANE.
By examining these common misunderstandings and mistakes you will learn practical steps
to avoid them. From the intake of the complaint to the closing of the investigation, this program addresses
the processes needed to maintain compliance with Texas regulations.

and investigation of suspected ANE in home health and hospice. This program presents the new rules & investigation
process identifed in Senate Bill 1880 that was passed last legislative sessions. SB 1880 provides for the Texas
Department of Family and Protective Services to be the state agency with primary
jurisdiction over ANE investigations. Examined is how these investigations have changed since the
implementation of SB 1880 and what has not changed. By examining common mistakes that agencies make with
regard to ANE complaints you will learn practical steps to avoid them. From the intake of the complaint to the

Program Description:The failure to timely report A&E to the Department of Family and Protective Services a
nd to DADS remain one of the higher range license violations among home and community care service agencies in Texas. There are several reasons for this.
First, many times management staff have the misunderstanding that they must make a judgment call as to whether or not an allegation has been made.
Second, middle management staff and delivery staff usually fail to understand how to dignify an allegation estimate.
This presentation examines final orders from the Texas Health and Human Services commission dealing with allegations of
abuse, neglect and exploitation. By examining these cases, as opposed to discussing a hypothetical story, the viewer will gain a better
understanding of what goes wrong when an allegation of abuse, neglect and exploitation is not timely reported to the authorities.

Program Description: Most home health and hospice providers have one or two experiences at buying or
selling an agency. This can make the task daunting, confusing, frustrating, and just downright confounding. This presentation reveals an
open discussion between an experience mergers and acquisitions broker and an experienced mergers and acquisitions attorney in the home health and hospice fields. Both discuss:
why someone might want to sell their agency
why someone might want to buy an agency
when someone might not want to sell their agency
when a potential purchaser may decide if a particular agency is worth what the seller wants
While they discuss the regulatory ins and outs of the process, they will also discuss the practical side in making a decision to either buy or sell an agency.

certified as a Merger & Acquisition Master Intermediary. He also is a Certified Merger & Acquisition Professional.
Mark has national responsibility for home health care, home care, hospice, and private duty markets. He and his team are responsible for the
comprehensive representation of each client throughout the entire transaction process including pre-market due diligence, buyer development, marketplace presentation,
deal negotiation, contract finalization, and closing. He has represented a full range of sellers including public companies, large regional home health & hospice
corporations, multi-division home health care conglomerates, and private duty agencies including both for-profit and not-for-profit entities.
Mark graduated from Fordham University with honors in Economics and Finance. He is an Advisory Board member of the Home Health Finance
Management Association, served on the board of the Home Care Technology Association of America, and has chaired the Southeast AeA Sales & Marketing assembly.
