Program Description:Texas Home and Community Support Service Agencies (HCSSAs) have experience with preparing, implementing, and
being surveyed upon Emergency Preparedness Plans. However, now certified home health and hospice agencies also have Federal Emergency Preparedness requirements with which to comply.
This presentation seeks to compare and contrast the Texas and Federal requirements and offer suggestions for how certified agencies can augment their current plans to
remain compliant with minimum rework. Each agency will understand what requirements they must meet based upon their category(ies) of licensure.
Obtaining policies and procedures and a plan is one thing, being prepared for survey by understanding and implementing your plan is the next step.
This presentation will illuminate the checklist agencies can use to assure they are survey ready regarding Emergency Preparedness.
More importantly, they will have taken important steps to insure the safety and stability of their clients, their staff members, and their companies.

Program Description: The number of investigations and inspections by regulatory
surveyors has steadily increased over the past few years. In 2018, the Texas Health and Human Services Commission conducted
4,500 visits and contacts of home and community support services agencies. To help home health and hospice providers prepare
for such a visit, this program provides guidance on how to avoid the most frequent deficiencies cited by the state agency.
Examples of survey findings with the rationale for the finding of deficiencies are highlighted. Part 1 includes a review of the survey process and a review of data from the
Texas HHS Annual Report Regarding LTC Regulatory - published August 2019 - and then presents details on deficiencies for Home Health; Part 2 presents deficiencies for Hospice.

Program Description: The current void of collaborative care between
the primary care physician’s practice, inpatient care, and home health is
costly for the U.S. healthcare system and can result in poor outcomes. Home health and primary care in a patient-centered
medical home (PCMH) has a role in addressing this need through a new approach with better care-
team collaboration.
PCMH is a patient-centric model of healthcare delivery based on ongoing, personal relationships
between patients, physicians, and healthcare teams, which includes the patient as a team member. In this program a primary care physician
describes how a Texas physician practice transformed into a medical home, adopted PCMH
strategies and partnered with hospitalists and preferred home health partners to improve care coordination,

About the Presenter: Nusrat (Ness) Khan, MD, MBA, FAAP, is
the Medical Director of MedPeds Medical Clinic, PA in Weatherford, TX. He
completed his early education, college, and medical school in North Carolina and completed specialty training
and board certification at the Cleveland Clinic Foundation in Cleveland, Ohio. He is double board certified
in Internal Medicine and Pediatrics. His love of teaching manifested early in his career; he has received
teaching awards at Cleveland Clinic Foundation, and at University of North Texas Health Sciences Center.
Dr. Khan remains a strong believer patient centered medical care model. For the last 2 years he has being

About the Presenter: Dr. Ket Davis is a native of Shreveport, Louisiana. She received her Bachelors of Nursing Science degree from
Grambling State University in 2004. She attended Grambling State University from 2010-2012 where she completed her Master of Nursing Science with a concentration in Family Practice.
She is a board-certified family nurse practitioner since 2012. Dr. Davis relocated to Fort Worth, Texas in 2012 where she became staff at the John Peter Smith Hospital from 2012-2013.
She later began working at Metrocare University in 2013. Dr. Davis went on to finish her doctorate training at the University of Texas at Arlington (UTA) in 2017.
