Apryl Swafford began her nursing career in 1993, obtaining a BSN from Jacksonville State University in Jacksonville, Alabama.
Her first few years were spent learning nursing, trial by fire style, in a 16 bed ICU unit. She transitioned into home health in late 1997 working PRN and weekends while learning her way and has
spent the majority of the years since working home health in some capacity. She has worked as a field clinician/case manager, clinical manager and leader of a cardiopulmonary specialty program.
During her tenure at the last two positions, she was responsible for chart audits, OASIS review and coding plus management of a team of nurses, therapist and home health aides.
Apryl is certified as an OASIS Specialist - Clinical (COS-C), and is a credential holder of the Homecare Coding Specialist (HCS-D) for both ICD-9 and ICD-10. During her free time, she enjoys
her family (there’s a lot to enjoy with 4 children and 9 grandchildren), working on the family’s small farm and running.

Program Description:Texas Home and Community Support Service Agencies (HCSSAs) have experience with preparing, implementing, and
being surveyed upon Emergency Preparedness Plans. However, now certified home health and hospice agencies also have Federal Emergency Preparedness requirements with which to comply.
This presentation seeks to compare and contrast the Texas and Federal requirements and offer suggestions for how certified agencies can augment their current plans to
remain compliant with minimum rework. Each agency will understand what requirements they must meet based upon their category(ies) of licensure.
Obtaining policies and procedures and a plan is one thing, being prepared for survey by understanding and implementing your plan is the next step.
This presentation will illuminate the checklist agencies can use to assure they are survey ready regarding Emergency Preparedness.
More importantly, they will have taken important steps to insure the safety and stability of their clients, their staff members, and their companies.
No one can totally prevent disasters and emergencies, but focused preparation will help mitigate their impacts.

Program Description: In order to curtail the continued presence of fraud, waste, and abuse within
government programs, specifically home health and hospice, the OIG has implemented the use of data analytics to highlight providers who are outliers
in terms of billing trends. It is essential that providers become aware of how their data analytics will be observed by an outside party.
Along those same lines, emphasis is now being placed on provider-administered compliance programs. Successfully administered compliance
programs will be key if a provider’s data analytics ever come into question. Use this presentation to gain insight from the original Federal
prosecutors in the Dr. Roy case who successfully led the indictment for one of the largest fraud cases in home health.

About the Presenters:
Mindy Sauter, JD and Mike Elliott, JD are both former Assistant United States Attorneys
who investigated and prosecuted health care fraud throughout the United States.
Mindy is a graduate of Oklahoma State University and SMU law school. Before she was a federal prosecutor,
Mindy was an Assistant District Attorney in Dallas where she tried hundreds of cases and supervised felony prosecutions.
Mike is a graduate of the United States Naval Academy and the University of Miami School of Law.
Prior to working in both the Southern and Northern Districts of Texas with the Department of Justice, Mike was in private practice in New York City.
Their firm, Elliott Sauter, focuses on representing health care organizations and professionals in regulatory, compliance and criminal matters.

Medicare Administrative Contractors around the country. TPE audits are merely the latest iteration of prepayment reviews being conducted by CMS contractors.
While the government has placed some restrictions on their use, contractors continue to exercise great latitude in their scope and the length that they are
imposed. Similarly, contractors remain in control of postpayment audits conducted and suspension actions recommended to CMS. In this webinar,

About the Presenter:
J. Robert Parkey is a sixth-generation native of the Wichita Falls area, and
received his Doctorate in Medicine from the University of Texas Medical Branch in Galveston, and then completed his residency in Family Medicine
at the University of Oklahoma campus in Tulsa, where he served as Chief Resident. In 2010, he completed a Masters degree in Theological Studies
from the University of Dallas in Irving, with a personal emphasis in Biomedical Ethics.
Dr. Parkey practiced rural medicine in Henrietta, Texas for 17 years, but found a special love for the care of the elderly,
those with dementia, and for patients with terminal and life-limiting illnesses, expanding his medical specialization and joining the medical staff of
Hospice of Wichita Falls in 2008, then closing his rural practice in 2013. He is a member of numerous medical societies, including the American Academy
of Hospice and Palliative Medicine, the Society for Post-Acute and Long-Term Care, the American Geriatrics Society, the American Society for
Bioethics and Humanities, and the Catholic Medical Association.
His “spare time” is filled caring for his family ranch in Clay County, and trying to keep up with his wife Katie
(a Registered Nurse who helps him run his private practice and who serves in a number of volunteer capacities in the area) and their three
daughters. His hobbies include fishing and literature, and he is a member of the Wichita Falls Poetry Society, the Texas Poetry Society, and
the Academy of American Poets.
Dr. Parkey holds board certifications in Hospice and Palliative Medicine, Family Medicine, and board
certification by the Hospice Medical Director Certification Board. He also holds board certification from the American Board of
Post-Acute and Long-Term Care Medicine.

modified many therapy requirements including elimination of the 10-visit threshold, objective testing, and
ongoing re-assessment re]inement to name a few. Auditors have focused on the rehab elements of patient claims
as they produce denials for unskilled or unnecessary care delivery. The Affordable Care Act (ACA) bundling model
will focus on the rehab content in homecare programs as they seek greater efficiency under decreased utilization
to achieve cost reduction goals.
Therapy ST/LT goal production, skilled per visit documentation, patient compliance,
caregiver involvement, and discharge planning and management are all addressed by a PPS-compliant, objective test-based
therapy evaluation. Clinical case examples will outline the good and bad in current therapy documentation, with strategies
