Continuing Education programs are offered for (1) a designated Administrator or Alternate who must comply with TAC §558.260, and (2) a prior administrator or alternate not currently designated but maintaining compliance with TAC §558.260, and (3) anyone who wants to improve their knowledge on a broad range of topics specific to TX HCSSAs.
Continuing Education programs come from TAHC&H conferences & events to address a broad range of topics specific to Texas HCSSAs administrative responsibilities. Programs are added throughout the year as each conference/event concludes. All programs provide TX HCSSA Administration CE credits and many also provide Nurse CE. and health services. In her state career, she analyzed state and federal legislation for impact to providers and consumers, promulgated and interpreted state regulations, developed and delivered
continuing education and training to providers and surveyors, and provided regulatory guidance to both industry representatives and leaders and state of Texas surveyors and investigators.
Rosalind is also a subject matter expert for federal standards and possesses a keen understanding of accreditation for home health and hospice care. Rosalind is well-versed
in the regulations for the settings where home care is delivered including nursing facilities, assisted living facilities, intermediate care facilities for individuals with developmental disabilities,
and independent living settings.
Rosalind has been a public speaker for home care and hospice since 2000; and she is now an independent healthcare consultant.

You are not alone! Hospice agencies across the nation are surprised that their comprehensive assessment composite, as well as some of the individual scores are lower than expected
per CMS data. Did you know that even if you answer positively to some of these process measures, CMS has "Validation questions" embedded in the HIS to ensure your answers represent
the measure, as CMS defines it. Literally, you may say "Yes, we screened the patient for pain"- but if any of the following questions - date, rating or type of standardized tool are
not answered correctly, per the measure, CMS will change your answer to "No, we did not screen the patient for pain". This presentation uncovers these validation questions, and provides

perspective are key when selecting assessment tools and interpreting findings, as well as when making decisions around medical necessity,
dosage, and interventions. Routines and priorities unique to the individual child/family are particularly important for children with chronic
or progressive conditions, given their complexity, and best practice for gathering and utilizing this vital information is addressed.
Additionally, perspective and language around goal development and collaboration with caregivers is explored. There are times in the
life of a child with a chronic or progressive condition where strategic use of the Maintenance Level / Prevent Deterioration frequency/dosage

J. Robert Parkey is a sixth-generation native of the Wichita Falls area, and
received his Doctorate in Medicine from the University of Texas Medical Branch in Galveston, and then completed his residency in Family Medicine
at the University of Oklahoma campus in Tulsa, where he served as Chief Resident. In 2010, he completed a Masters degree in Theological Studies
from the University of Dallas in Irving, with a personal emphasis in Biomedical Ethics.
Dr. Parkey practiced rural medicine in Henrietta, Texas for 17 years, but found a special love for the care of the elderly,
those with dementia, and for patients with terminal and life-limiting illnesses, expanding his medical specialization and joining the medical staff of
Hospice of Wichita Falls in 2008, then closing his rural practice in 2013. He is a member of numerous medical societies, including the American Academy
of Hospice and Palliative Medicine, the Society for Post-Acute and Long-Term Care, the American Geriatrics Society, the American Society for
Bioethics and Humanities, and the Catholic Medical Association.
His “spare time” is filled caring for his family ranch in Clay County, and trying to keep up with his wife Katie
(a Registered Nurse who helps him run his private practice and who serves in a number of volunteer capacities in the area) and their three
daughters. His hobbies include fishing and literature, and he is a member of the Wichita Falls Poetry Society, the Texas Poetry Society, and
the Academy of American Poets.
Dr. Parkey holds board certifications in Hospice and Palliative Medicine, Family Medicine, and board
certification by the Hospice Medical Director Certification Board. He also holds board certification from the American Board of
Post-Acute and Long-Term Care Medicine.

Home health documentation has never been under more scrutiny. Six states now operate under the Review Choice Demonstration, ADR volumes from Palmetto GBA TPE rounds remain elevated, Noridian SMRC project reviews continue to expand, and UPIC investigations carry broader authority than ever. Across every one of these programs, reviewers ask the same question: does the record support what was billed?
This session illuminates exactly what reviewers look for — and exactly where agencies most often fall short. We will spend dedicated time on the Review Choice Demonstration, including Palmetto GBA's four-question framework for establishing medical necessity, the documentation bundle required for Pre-Claim Review affirmation, and the most common non-affirmation reasons paired with specific documentation fixes. We will then walk through the anatomy of a typical denial, the four most common documentation disconnects between OASIS, the 485, visit notes, and the face-to-face encounter, high-risk OASIS items, the Golden Thread that ties assessment to justification, and the building blocks of a defensible record — including pre-bill QA, education that actually changes clinician behavior, and AI-assisted audit tools.
