Continuing Education programs are offered for (1) a designated Administrator or Alternate who must comply with TAC §558.260, and (2) a prior administrator or alternate not currently designated but maintaining compliance with TAC §558.260, and (3) anyone who wants to improve their knowledge on a broad range of topics specific to TX HCSSAs.
Continuing Education programs come from TAHC&H conferences & events to address a broad range of topics specific to Texas HCSSAs administrative responsibilities. Programs are added throughout the year as each conference/event concludes. All programs provide TX HCSSA Administration CE credits and many also provide Nurse CE. All programs are à la carte, that is, you have the flexibility to browse and order programs that suit your specific needs and are applicable to your role and responsibility. About the Presenter(s): Leslie Heagy is Registered Nurse who joined Healthcare Provider Solutions, Inc. (HPS), as the Clinical Services Manager,
in June 2017. Leslie is part of the clinical consulting team providing support and solutions to home health and hospice agencies across the country to achieve regulatory compliance.
In 2022, Leslie was promoted to Director of Clinical Services over both home health and hospice clinical consulting.
Leslie’s commitment to service and compassionate leadership first took root 21 years ago when she began providing care to patients as a hospice nurse after
receiving her nursing degree from Ivy Tech Community College in Columbus, Indiana. Throughout her nursing career, she has worked in various capacities within the home health,
hospice and private duty industries. Her experience includes successfully opening a Medicare/Medicaid licensed hospice start-up program, leading multidisciplinary teams
in various locations to being part of the team to open a new hospice inpatient unit. She has educated hospitals, skilled facilities, assisted living facilities
and the community on the benefits and requirements for home health and hospice services. Leslie’s extensive Federal and State Regulatory knowledge, along
with her vast clinical experience motivates her to continually seek solutions to the barriers of healthcare in all settings.
Leslie has operational experience in the survey process for Medicare/Medicaid licensed agencies and has assisted agencies in developing/implementing
corrective action plans. Her experience includes clinical consulting, performing clinical chart reviews, onsite billing audit reviews, provider education, developing and
implementing quality and performance improvement programs for home health and hospice agencies.

Program Description: This presentation covers the details of the Value-Based Insurance Design (VBID) Model – Hospice Benefit Component.
Medicare Advantage (MA) has long had coverage for home health patients, but there has never been coverage for Hospice under the MA benefit. VBID is a demonstration that will begin
January 1, 2021. There are 9 Medicare Advantage Organizations (MAO) that will be participating and have coverage in multiple states. In addition to Hospice care there is an expansion
of Palliative Care services that will be offered to patients through the MAO. CMS is testing how the hospice benefit component can improve beneficiary care through greater care coordination,
reduced fragmentation, and transparency in line with recommendations by the Office of Inspector General (OIG), the Medicare Payment Advisory Commission (MedPAC) and others.
CMS will require that MAOs provide beneficiaries with broad access to the complete original Medicare hospice benefit. This presentation covers all known aspects of the demonstration and prepares
hospices for the reality that Medicare Advantage is now in the Hospice world.

Program Description: Major scrutiny of the Hospice Benefit is well underway nationwide. This program reviews the details of both programs and what Hospices can expect.
The Special Focus Program (SFP), will monitor hospices identified as poor performers based on selected quality indicators. Hospices selected for the SFP will be under additional oversight to enable continuous improvement.
This required program includes the development and implementation of enforcement remedies for noncompliant hospice programs, as well as procedures for appealing determinations regarding these remedies.
These enforcement actions can be imposed instead of, or in addition to, termination of the hospice program’s participation from the Medicare program. Through the PPEO, new providers or suppliers are
subject to prepayment medical reviews designed to address improper billing practices right out of the gate. This program is for newly certified hospices as well as change of ownership and a must-view!

of SHP’s industry leading analytics platform and benchmark data. Prior to SHP, Chris was the CFO at the VNA of Boston and had worked at Partners Health Care at Home as CFO and CEO. He received
a B.A. in Public Health from Tufts University and an M.B.A. from the Graduate School of Management at Boston University concentrating in Health Care Finance.
About the Presenter(s): Angela Huff With over 35 years of experience as a registered nurse, Angela has spent nearly 25 years in executive
leadership in home health and hospice operations as well as software development in the post-acute space. In addition, she has global experience in the community care space that
includes the United Kingdom, Finland, Canada, and Australia. Angela performs operations improvement assessments and compliance-focused reviews of home health and hospice documentation,
including audits and appeals related to the OIG and program integrity contractors as well as compliance risk assessments related to mergers and acquisitions.
She is actively involved in the educational and advocacy activities of NAHC and NHPCO and is a member of the HHFMA Innovation, NAHC Education and Women in
Leadership Committees. Angela has been a presenter at NAHC and NHPCO, as well as other industry national and state organizations and has been published in the Nursing Administration
Quarterly and other industry publications. Before entering the post-acute industry, she served in acute care management positions and was a Certified Critical Care Registered Nurse
and a Certified Registered Nurse Infusionist.

She gained her hospice operations experience as the President/CEO of San Diego Hospice for 10 years prior to her current position with VITAS. Prior to hospice, Holly worked in home health. Dr. Swiger has previously served 8 years as the National Hospice & Palliative Care Organization’s Board of Directors. Holly is a member of the NHPCO’s Regulatory Committee, Managed Care Advisory Committee, CMS Regional Task Force and Chairs the Medicaid Task Force. Dr. Swiger also serves on the Public Policy Committee for the Hospice associations in Texas, California, Kansas and Missouri. Rebecca McMinn, BSN, RN, MBA brings twenty-five years of experience in the hospice field to the Texas based home health and hospice consulting firm, HealthCare ConsultLink. With over 35 years in nursing, the majority of her experience has been in the ICU setting and in hospice. Annette is a registered nurse, with a Master’s in Health Care Administration, practicing since 1990. For over a decade she worked with the CMS Home Health and Hospice intermediary, Cahaba GBA and CGS, where she provided review of, and education on home health and hospice documentation. She worked as a specialist in the Medical Review, Appeals, and Provider Education departments, and assisted with cross-MAC quality assurance and training.
Annette then founded Provider Insights, Inc. providing consultative support and provider education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers across the nation with Survey Preparation, ADRs and appeals. Today she continues to marry together her experience in the “real world” and the inside knowledge of Medicare to ensure providers can meet the requirements of CMS, and ensure compliant, efficient operations.

Home Health reforms continue in terms of programming and reimbursement changes as outlined in the 2026 Final Rule.
Providers seeking success will face new challenges when addressing HHCAHPS and VBP changes, with cuts in reimbursement occurring simultaneously.
The result is a Home Health landscape considerably different from what most of us have worked under during the PPS and early PDGM era.
When traditional operations and programming approaches fail to deliver desired outcomes under PDGM and in areas such as VBP and HHCAHPS,
many Providers find themselves behind the eight-ball in terms of episodic margins. But the Volume-2-Value (V2V) element of PDGM combines with
the Pay-for-Performance (P4P) identity of VBP to outline a clinical path to address Home Health Fiscal margins.
