Over the last year, CMS has been digging into their proverbial treasure chest of regulations to implement new rules for Hospices.
After a strong push-back from industry associations, the new Hospice Special Focus Program (SFP) has been placed on hold for the time being.
The new HOPE tool is following the HHA OASIS structure to provide new assessment time points and measures based on new data items.
CAHPS Hospice now has fewer questions including a new option for web-based surveys, a mode in use by Medicare Advantage patients.
There is a lot to unpack with all these changes for FY 2025. Join our speakers as they review these planned
changes and how Hospices should be evaluating the best way to implement.

With over 35 years of experience as a registered nurse, Angela has spent nearly 25 years in executive
leadership in home health and hospice operations as well as software development in the post-acute space. In addition, she has global experience in the community care space that
includes the United Kingdom, Finland, Canada, and Australia. Angela performs operations improvement assessments and compliance-focused reviews of home health and hospice documentation,
including audits and appeals related to the OIG and program integrity contractors as well as compliance risk assessments related to mergers and acquisitions.
She is actively involved in the educational and advocacy activities of NAHC and NHPCO and is a member of the HHFMA Innovation, NAHC Education and Women in
Leadership Committees. Angela has been a presenter at NAHC and NHPCO, as well as other industry national and state organizations and has been published in the Nursing Administration
Quarterly and other industry publications. Before entering the post-acute industry, she served in acute care management positions and was a Certified Critical Care Registered Nurse

Program Description: This presentation covers the details of the Value-Based Insurance Design (VBID) Model – Hospice Benefit Component.
Medicare Advantage (MA) has long had coverage for home health patients, but there has never been coverage for Hospice under the MA benefit. VBID is a demonstration that will begin
January 1, 2021. There are 9 Medicare Advantage Organizations (MAO) that will be participating and have coverage in multiple states. In addition to Hospice care there is an expansion
of Palliative Care services that will be offered to patients through the MAO. CMS is testing how the hospice benefit component can improve beneficiary care through greater care coordination,
reduced fragmentation, and transparency in line with recommendations by the Office of Inspector General (OIG), the Medicare Payment Advisory Commission (MedPAC) and others.
CMS will require that MAOs provide beneficiaries with broad access to the complete original Medicare hospice benefit. This presentation covers all known aspects of the demonstration and prepares
hospices for the reality that Medicare Advantage is now in the Hospice world.

Program Description: With the implementation of Hospice Quality Reporting Measures and the CAHPS Hospice Survey,
hospice staff and managers must understand the impact of their daily interactions with patients/families and how they may directly influence public
reporting measures (and future star ratings). This presentation provides a brief overview of these regulatory requirements and engages the participants
in exploring each of the questions and the direct impact that staff interaction may have on HIS and CAHPS Hospice Survey responses. This presentation utilizes
case scenarios and group discussion to demonstrate exceptional customer service behaviors that hospice staff can use in daily practice to improve in these measures.

Kimberly Skakhan is Director of Compliance, Regulatory & Quality at Simione Healthcare
Consultants with more than 30 years of clinical, management and consulting experience in home health and hospice. Kim is nationally recognized and highly respected by
state, federal and industry leaders. She has acted as a liaison to state and federal regulatory agencies and legislators to facilitate collaboration regarding issues
affecting the home care and hospice industry. Kim currently serves as a member of the Regulatory Committee of the National Hospice & Palliative Care Organization (NHPCO)
and the Education Committee for the National Association for Home Care and Hospice. She is also a member of the Connecticut Association for Healthcare at Home Kim is a
certified consultant by the Accreditation Commission for Health Care(ACHC) for home health, hospice and private duty care. An AHCC-accredited home care coding specialist
and certified OASIS reviewer(COS-C), Kim earned her B.S. degree in nursing from Southern Connecticut State University, and her M.S. degree in nursing administration from
the University of Connecticut (UCONN), where she has also served as adjunct faculty for the School of Nursing.

Melynda Lee, a member of the Simione team since 2016, is an expert in home care and hospice sales,
marketing and referral management, communications, public relations, administration and strategic planning. Melynda previously served as a home health and
hospice leader in Florida and Georgia for 8 years, and is a former board member and education committee member for the Georgia Hospice and Palliative Organization.
She is a co-founder of the Greater Atlanta Hospice-Veteran Partnership. She is an expert trainer of home care and hospice professionals, and speaker at state and
national association conferences. The recipient of a Digital Marketing Certificate from Cornell University, Melynda excels n assessment of marketing and
communications programs, brand and marketing communications, and client assistance to measure and improve competitive strategies for growth. She is also a

Program Description: Regulatory Compliance is of grave concern always, but especially during times of
heightened scrutiny by medical reviewers. CMS’ has stated concerns about the sources of clinical information for hospice certifications of terminal
illness (CTI) and other recent reminders from CMS emphasizes the importance of compliance with CTI and Medicare election statement requirements. These
items will be part of every medical review and there is a new focus on them during Medicare surveys. The comprehensive assessment of the patient is
the ground work for the care to be provided and requires additional attention by most agencies. This program delves deep into the requirements for
the CTI, Medicare election statement and comprehensive assessment and their impact on operations. It also covers other regulatory and billing necessities
such as the plan of care, Notice of Election (NOE), and current medical review topics.
Program Learning Outcome: Participants will actively engage in the learning activity and indicate an
intent to change their practice by implementing best practices in the comprehensive assessment of the terminal patient to contribute to optimal patient outcomes.
