Cypress won the Better Business Bureau’s Business Ethics award in 2013, and in 2018. Bob hosts a radio show called “Health Futures, Taking Stock in You.” This program airs every Friday from Noon to 1:00 pm on Money Radio 1510 AM, 105.3 FM. In addition, he writes a monthly column called “Aging Today” for the Jewish News of Greater Phoenix, and Lovin Life after 50 newspapers. In March 2017 Bob was appointed to the Governor’s Advisory Council on Aging by Arizona Governor Doug Ducey. This was the first time in the Council’s 40 years that a home care/home health care agency owner/manager has served on the Council. On September 11, 2019 Bob was selected the winner for the Home Health Care News Future Leader Award - Recognizing up-and-coming leaders elevating the home health industry. Michelle Cornelius-Toft earned her Master of Social Work in 1997 from Arizona State University, and in 2011 her Master of Education in Human Resource Development from the University of Illinois. The majority of Michelle’s work experience has been with people diagnosed with dementia, either in their homes or in memory care facilities. Michelle Toft is currently the Director of Memory Care Programs at Cypress HomeCare Solutions in Phoenix, AZ and a Program Specialist for Honor Care Network out of San Francisco, CA. She is a public speaker at national conferences and provides consulting services to help others implement dementia programs within their own organization. At Cypress HomeCare Solutions and Honor Care Network she has implemented a dementia training program for their caregivers, a family support and education program for the family members of those diagnosed, and provides public educational sessions. Now is the time to ramp up for the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based) quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. However, despite having large amounts of data, providers are generally faced with the challenge of not knowing how to leverage the data beyond basic reporting. Moreover, in the home and community care sector in particular, visit, operational information, and clinical data is often stored in paper format -- or if is it digital, resides in siloed databases. These system deficiencies make it near impossible to gain a holistic view of operations and eliminates the ability to “pivot” data. The catalyst for this change is to be obsessed about data-driven decision making. The rise of business intelligence (BI) is unlocking actionable insights that will dramatically improve home and community care organizations. Sherry Teague, a native Floridian, is the CFO and co-owner of K&K Health Care Solutions. Sherry has been the driving factor in both her and Dee’s movement from employment to ownership – first with a contract rehabilitation company (Integrity Rehab, Inc.) that provided physical therapy, occupational therapy and speech-language pathology services to local home health agencies in Marion and Citrus counties, and then into their own home health agency, founded in 2003. In 2009, Sherry was honored with the Marion County Chamber of Commerce “Small Business Person of the Year” Award. Sherry is dedicated to taking the pain out of continuing education for health care clinicians and administration by leveraging technology and meeting customer needs. Sherry has a strong sense of community service, as evidenced by her volunteer status as an athletic trainer at Citrus High School for over 13 years, as well as her membership with Rotary International that exemplifies service above self. Sherry received her athletic training certification from Valdosta State University (GA) in 1989, and her Masters in Exercise and Sport Science degree from the University of Florida in 1991. She completed her A.S. degree at St. Pete Jr. College in St. Petersburg, FL, to become a physical therapist assistant in 1998. About the Presenter(s):

In 2022, Leslie was promoted to Director of Clinical Services over both home health and hospice clinical consulting. Leslie’s commitment to service and compassionate leadership first took root 21 years ago when she began providing care to patients as a hospice nurse after receiving her nursing degree from Ivy Tech Community College in Columbus, Indiana. Throughout her nursing career, she has worked in various capacities within the home health, hospice and private duty industries. Her experience includes successfully opening a Medicare/Medicaid licensed hospice start-up program, leading multidisciplinary teams in various locations to being part of the team to open a new hospice inpatient unit. She has educated hospitals, skilled facilities, assisted living facilities and the community on the benefits and requirements for home health and hospice services. With the negative reimbursement impacts to hospitals and a likely HHA pay-for-performance measure, agencies need to continually look at their data to understand the relationships of patient characteristics and the utilization of services provided to improve their scores. Using data from the SHP (Strategic Healthcare Programs) database, we will analyze the different characteristics and agency profiles behind hospital readmissions. The data provided during this session will help agencies focus and refine efforts that lead to better performance.
