Michelle Cornelius-Toft earned her Master of Social Work in 1997 from Arizona State University, and in 2011 her Master of Education in Human Resource Development from the University of Illinois. The majority of Michelle’s work experience has been with people diagnosed with dementia, either in their homes or in memory care facilities. Michelle Toft is currently the Director of Memory Care Programs at Cypress HomeCare Solutions in Phoenix, AZ and a Program Specialist for Honor Care Network out of San Francisco, CA. She is a public speaker at national conferences and provides consulting services to help others implement dementia programs within their own organization. The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes. Even though CMS is omitting the TNC from its programs and replacing it with the Discharge Function score, the TNC scores are still important through payment year 2026. Now is the time to ramp up for the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based) quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. PPH will include hospitalizations (both ACH and LTCH) and observation stays. The DTC measure will include readmissions for up to 31 days after discharge. Bias, as defined by the American Nurses Credentialing Center’s Commission on Accreditation (ANCC COA/LSNA) is the “tendency or inclination to cause partiality, favoritism, or influence.” Commercial bias may occur when a CNE activity promotes one or more product(s) (drugs, devices, services, software, hardware, etc.). This definition is not all inclusive and participants may use their own interpretation in deciding if a presentation is biased. The ANCC COA is interested in the opinions and perceptions of participants at approved CNE activities, especially in the presence of actual or perceived bias in continuing education. Therefore, ANCC invites participants to access their “ANCC Accreditation Feedback Line” to report any noted bias or conflict of interest in the education activity.
