Program Description:HEART is a standardized comprehensive assessment tool that CMS will
require all Medicare certified hospices to use. While we do not have an exact tool or an exact date of implementation, this program
does discuss what is happening now with HEART, how it will be used, what might be in the tool, and a possible timeline. Lessons learned
from other provider types about mandated standardized assessments is shared with suggestions for preparation specific to hospices.
The presentation incorporates lessons learned so far in the hospice quality reporting program.

development and delivery since the introduction of PDGM. Along these lines, Value-based Purchasing was installed for Home Health,
connecting payment levels to outcomes for the first time in our industry. A review of the requirements for the VBP bonus reveals
outcomes rarely seen to date in HHCAPS, Readmission, Timeliness, and Clinical Outcomes. In addition, HH Readmissions must be in the

About the Presenter(s): Kay Hollers is well known to TAHCH audiences as she has been an active member or associate member
since her return to Texas in 1985. Kay holds a Bachelor of Science degree from the University of Texas with a major in Nursing and a Master’s Degree in Public Health
from the University of Texas School of Public Health. She also earned a post-master’s Specialist Certification in Health Care Evaluation from Wayne State University
College of Nursing in Detroit. Kay taught Public Health Nursing at Michigan State University and has practiced in and consulted with home health and hospice agencies
large and small, public and private. She currently is a consultant with HealthCare Consultlink. Kay likes to share her experiences and knowledge with audiences and

About the Presenter: Linda Parker’s career in home health began in 1976 as a field nurse. She has been a part
of the HealthCare ConsultLink team for over 17 years and is certified in OASIS (COS-C), ICD-10 Coding (HCS-D) and has a love for Quality Assessment and Performance Improvement (QAPI).
Her passion for quality care has led her to work directly with agencies as a consultant and educator so that the quality of care, as well as patient outcomes, are improved.

Program Description: All agencies believe they provide
quality service in an effective, efficient, and economically sound manner. But proving and backing up
the assertions requires a Quality Assessment and Performance Improvement (QAPI) program that
quantifies results in a measurable way. This program reviews the regulatory requirement for
QAPI program including selection of committee members, development of an effective QAPI policy
and plan, minimal requirements set forth by the state, and activities that can be performed,
measured, and quantified from point to point of review. It also provide updates
for the new QAPI condition and agency requirements applicable to Medicare certified home health
agencies to be implemented on January 8, 2018. The presenter also provides a sample policy and
procedure guide for the new requirements.

Chief Executive Officer of MJS & Associates, LLC. She has more than 30 years’ experience in the health care field.
Ms. Spears has owned and operated a multimillion dollar home health care company as well as consulted
for over 300 homecare providers in 25 states. As a full time consultant for the past 15 years, Ms. Spears
has successfully merged clinical and financial strategies into successful operational plans for
numerous providers nationwide. Jan serves as a Medicare coverage expert for health care facilities
who are undergoing audits by federal contractors through all levels of appeals. She has authored
several publications and heads a team of more than 25 consultants at MJS & Associates, LLC.

Program Description: This program helps you prepare for audit
scrutiny. The presenter has had a front-row seat as audits performed by Fiscal
Intermediaries served as a warm-up for the RAC (Recovery Audit Contractor) era.
Having analyzed audited cases from multiple agencies in multiple states, this
program describes denied care in clinical terms that can help providers
identify claim concerns that exist in their programs. Specific dissections of
partial denials illustrate areas where clinical improvements can help agencies
re-wire care delivery for future stability and success. This program is
important for all levels of home health professionals.
Program Objectives: Upon completion of this program, the viewer will be able to:
Identify the challenges facing the Home Health industry as they confront more stringent regulatory constraints and audits
