In 2010, he completed a Masters degree in Theological Studies from the University of Dallas in Irving, with a personal emphasis in Biomedical Ethics. Dr. Parkey practiced rural medicine in Henrietta, Texas for 17 years, but found a special love for the care of the elderly, those with dementia, and for patients with terminal and life-limiting illnesses, expanding his medical specialization and joining the medical staff of Hospice of Wichita Falls in 2008, then closing his rural practice in 2013. He is a member of numerous medical societies, including the American Academy of Hospice and Palliative Medicine, the Society for Post-Acute and Long-Term Care, the American Geriatrics Society, the American Society for Bioethics and Humanities, and the Catholic Medical Association. His “spare time” is filled caring for his family ranch in Clay County, and trying to keep up with his wife Katie (a Registered Nurse who helps him run his private practice and who serves in a number of volunteer capacities in the area) and their three daughters. His hobbies include fishing and literature, and he is a member of the Wichita Falls Poetry Society, the Texas Poetry Society, and the Academy of American Poets. However, now certified home health and hospice agencies also have Federal Emergency Preparedness requirements with which to comply. This presentation seeks to compare and contrast the Texas and Federal requirements and offer suggestions for how certified agencies can augment their current plans to remain compliant with minimum rework. Each agency will understand what requirements they must meet based upon their category(ies) of licensure. Obtaining policies and procedures and a plan is one thing, being prepared for survey by understanding and implementing your plan is the next step. This presentation will illuminate the checklist agencies can use to assure they are survey ready regarding Emergency Preparedness. More importantly, they will have taken important steps to insure the safety and stability of their clients, their staff members, and their companies. No one can totally prevent disasters and emergencies, but focused preparation will help mitigate their impacts. Medicare Advantage (MA) has long had coverage for home health patients, but there has never been coverage for Hospice under the MA benefit until 2021 when the VBID demonstration began to include Hospice. There have been many changes to the Medicare Advantage Organizations (MAO) participating in 2024 as the demonstration has now been extended through 2030. The differences in coverage for an in-network vs. out-of-network provider will be reviewed and the allowance of MAO plans to force patients to elect IN-NETWORK ONLY. CMS requires that MAOs provide beneficiaries with broad access to the complete original Medicare hospice benefit. This program covers all known aspects of the demonstration and will prepare hospices for the reality that Medicare Advantage is now in the Hospice world to stay. Currently it is based on two composite scores for self-care and mobility (the TNC) along with improvement in dyspnea and oral medication management and discharged to community. This program takes a good look into the stand-alone OASIS items and describes how the TNC is calculated. It also discusses the future changes to the TPS with the omission of the TNC from the TPS altogether. The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes by omitting the TNC from its programs and replacing it with the Discharge Function score.
