Actionable VBP BI Dashboards: Understanding & Taking Action on KPIs That Lead to Success

[3624] Actionable VBP Business Intelligence Dashboards: Understanding and Taking Action on KPIs That Lead to Success
This presentation is a must for those wishing to streamline their operations and achieve VBP financial success. Presentation Info: TAHC&H Winter Conference 2022 | Dallas | Recorded for On-Demand Access Presentation Date | February 24, 2022 Program Description: Information overload describes the excess of information available to a person aiming to complete a task or make a decision; resulting in a poor decision or no decision being made. This often happens in the fast-paced world of home care operations. Leaders are inundated from metrics from multiple sources that are often not actionable and not concurrent. This leads to no action and poor outcomes. Understanding the value of KPIs is the first step to successful outcomes. Developing a business intelligence dashboard that contains the Metrics that Matter in the new world of Value Based Purchasing is the next and most critical step. Metrics should be easy to understand and lead operators to implement corrective action concurrently. Thus, turning a possible negative outcome into a positive one. Participants will walk away with an understanding of root cause analysis and multi contributing factors affecting metrics so that they can apply rapid corrective action measures. This presentation is a must for those wishing to streamline their operations and achieve VBP financial success. HHCAHPS: Patient Experience vs Patient Satisfaction and Why it matters in HHVBP

[3837] HHCAHPS: Patient Experience vs Patient Satisfaction and Why it matters in HHVBP
In-depth look at the HHCAHPS survey to learn how scores are interpreted, their impact, and how to improve your agency’s performance. Presentation Info: TAHC&H Event | Winter Conference | Fort Worth | Rich-Media Presentation Capture for Your Online On-Demand Access Presentation Date | February 22, 2024 Presentation Track | Medicare Home Health Program Description: With the introduction of the Home Health Value-Based Purchasing (HHVBP) Model, there's a direct financial implication for agencies based on their Home Health Care Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) measures. It's vital for home health providers to recognize that "Patient Experience" is as crucial, if not more so, than the conventional view of HHCAHPS as simply a "Patient Satisfaction" indicator. A detailed examination of HHCAHPS reveals that an impressive 17 out of 19 questions zero in on patient experience, underlining its increasing significance. This focus resonates with the trends we've seen in acute care settings in the past few years. Elevating your agency's HHCAHPS scores holds significance because it indicates not just patient contentment but can also catalyze enhancements in quality, influence reimbursement and financial support, establish a competitive edge, guarantee adherence to regulations, furnish ongoing insights, enhance staff involvement, and foster a culture of accountability and transparency. Ultimately, this endeavor contributes to improved patient care and outcomes. In this program you will receive insights into the specific HHCAHPS questions that influence patients' evaluations of clinicians. Actionable strategies are offered to help agencies enhance patient experiences, improve HHCAHPS scores, and cultivate a more robust relationship between patients and clinicians in today's healthcare environment. View this in-depth look at the HHCAHPS survey to learn how scores are interpreted, their impact, and how to improve your agency’s performance. What’s now and what’s new? Presentation Info: TAHC&H Event Administrator Conference | Rich-Media Presentation Capture | Online On-Demand Access Presentation Date | November 8, 2023 Program Description: What’s now and what’s new? The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes. Even though CMS is omitting the TNC from its programs and replacing it with the Discharge Function score, the TNC scores are still important through payment year 2026. Now is the time to ramp up for the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based) quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. PPH will include hospitalizations (both ACH and LTCH) and observation stays. The DTC measure will include readmissions for up to 31 days after discharge. Lisa discusses what actions can be taken now to improve those numbers.
