officer at a medical device manufacturer. She also served as an outside compliance expert to a hospital and a clinical laboratory that were under Corporate Integrity Agreements.

She worked as a specialist in the Medical Review, Appeals, and Provider Education departments, and assisted with cross-MAC quality assurance and training.
Annette then founded Provider Insights, Inc. providing consultative support and provider education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers across the nation with Survey Preparation, ADRs and appeals. Program Description: Home health and its regulation in Texas is a changing landscape and the recently published common state violations for state fiscal year 2022 reflect the challenges faced by
agencies. This session will highlight the frequently cited areas of noncompliance for licensed home health agencies in Texas. In a review of the frequent citations, the presenter
will list possible mitigation strategies, required documentation, and pressure points for self-assessment. Attending this training will help the agency administrator prepare for
surveys and investigations by strengthening the understanding of State licensing standards and expectations.

Tony comes to Axxess with over 20 years of experience in the healthcare space, with a primary focus on home health. He spent 10 years at the LHC Group beginning his career as a

Program Description: What’s now and what’s new? The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes.
Even though CMS is omitting the TNC from its programs and replacing it with the Discharge Function score, the TNC scores are still important through payment year 2026. Now is the time to ramp up for
the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your
ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based)
quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. PPH will include hospitalizations (both ACH and LTCH) and observation stays.
The DTC measure will include readmissions for up to 31 days after discharge. Lisa discusses what actions can be taken now to improve those numbers.

but there are many challenges that have been with us since the beginning. Of 366 patients discharged from the hospital with a follow-up PCP appointment within two months:
64% used at least one med not ordered at discharge
