Program Description: CDI is rapidly making its way into the home health and hospice industry! The onslaught of medical reviews resulting in denial of
payment, significant overpayments, and even suspension of payments has pointed out one clear fact - a Clinical Documentation Improvement Program is tantamount to survival. The name is new;
the process is not. Agencies have been doing many pieces of CDI for years but have failed to formalize it into a robust CDI Program. Improving documentation to support skilled need,
coding, outcomes, quality initiatives, reimbursement is ongoing and is distributed among many roles in the office. But the time to formalize the program is NOW!
This presentation describes the process for setting up a CDI in your agency with the purpose being to improve patient outcomes, prevent denials, maintain survey readiness, improve the
integrity of documentation and coding process to ensure accurate reimbursement, protect the agency from liability and claims errors, improve Agency Star Ratings,
Value Based Purchasing Outcomes, and CAHPS while maintaining peace of mind in the agency's overall operation. CDI can be a part of the QAPI program, but

Jan Spears is Co-owner and Chief Executive Officer of MJS & Associates, LLC. She has more than 35 years’ experience in
health care administration and regulatory and reimbursement areas of practice. Ms. Spears has owned and operated a multimillion dollar home health care company as well as consulted for more
than 300 homecare providers in 25 states in the past 20 years. Ms. Spears has successfully merged clinical and financial strategies into successful operational plans for numerous
providers nationwide. She serves as a Medicare coverage expert for health care facilities who are undergoing audits by federal contractors through all levels of appeals. Since
2015, Spears has also provided testimony as a Medicare reimbursement and billing expert at criminal trials for physicians, home health and other provider types involved in
Medicare fraud litigation, including the largest physician fraud case in U.S. history. She has authored several publications including most recently the development of
process and forms for complying with upcoming pre-claim review initiatives. Always the advocate for home health and hospice and as a certified educator in the state,
Spears is a frequent guest speaker at stakeholder association meetings in Texas and surrounding states where she provides practical guidance to people in the

About the Presenter(s): Lynn Smith has over 28 years of clinical, management, and survey experience in the home health and hospice arenas. Lynn’s most recent position prior to joining SimiTree was as a full-time surveyor for home health, hospice, and palliative care for 9 years.
In her role as a Senior Manager, Lynn provides consultation and direction in maintaining agency licensure, accreditation, and coverage compliance requirements. Lynn conducts mock surveys and supports state, federal, and accreditation survey preparedness and plan of correction follow-up. Lynn assists organizations with staff training regarding Medicare eligibility and regulatory requirements and acts as a resource to clients and other SimiTree teams regarding state, federal, and accreditation compliance and regulatory updates.
Lynn is a CHAP Certified Consultant and an ACHC Certified Consultant. Lynn earned her B.S. in nursing from Indiana University of Pennsylvania and her M.S. degree in nursing administration from the University of Phoenix.

About the Presenter(s): Rosalind Nelson-Gamblin recently retired from a 33-year career in state government at the Texas Health and Human Services Commission,
where she was a senior policy specialist for regulation of the home health, hospice, and personal assistance services industry including supportive palliative care, assisted living, and day activity
and health services. In her state career, she analyzed state and federal legislation for impact to providers and consumers, promulgated and interpreted state regulations, developed and delivered
continuing education and training to providers and surveyors, and provided regulatory guidance to both industry representatives and leaders and state of Texas surveyors and investigators.
Rosalind is also a subject matter expert for federal standards and possesses a keen understanding of accreditation for home health and hospice care. Rosalind is well-versed
in the regulations for the settings where home care is delivered including nursing facilities, assisted living facilities, intermediate care facilities for individuals with developmental disabilities,
and independent living settings.
Rosalind has been a public speaker for home care and hospice since 2000; and she is now an independent healthcare consultant.

About the Presenter(s): Tony Meluzio is the Senior Content Manager for Axxess, the leading technology innovator for healthcare at home.
Tony is responsible for the design and development of the Axxess Training and Certification Program. During his tenure, he has been able to advance Axxess’ educational products all
while decreasing seat time for training while improving outcomes.
Tony comes to Axxess with over 20 years of experience in the healthcare space, with a primary focus on home health. He spent 10 years at the LHC Group beginning his career as a
clinician and then advancing to corporate training where he designed and developed workforce management solutions, decreased onboarding time and turnover, created OASIS training,
clinical specialty programs and survey readiness education. Tony also served as a consultant to other teams in the organization, detailing how to better deliver content and improve
employee engagement. Since 2018, he has served as a professional consultant for EMR system optimization, survey readiness and documentation review.
Tony is a volunteer WVU Extension Master Gardener Program, where he leads community education efforts related to agricultural practices, sustainable living, raised bed gardening
and a seed bank. He is also active in his local rotary chapter and the Pay It Forward Program, where he helps collect and distribute gently used items to people in need.
Tony holds degrees in physical therapy and healthcare administration from Fairmont State University and a master’s degree in instructional design and technology from West Virginia University.
