Program Description:The next step to understanding the Emergency Preparedness Regulations is to determine how to prepare and present evidence of your compliance.
In this presentation Kay Hollers functions as a surveyor and Jennifer Gibson Osburn functions as an agency administrator. Agencies will be able to learn what to expect from the
surveyor and responses from the agency administrator that satisfy the survey process. They also present strategies agencies can use to be comfortable they have put their best foot forward at survey time.

Health Care of Sparta Community Hospital since its inception in 1996. Cheryl is responsible for developing, implementing and sustaining the business plan for the hospital
based home health agency known as At-Home Health Care. The agency under her direction has a history of multiple deficiency free surveys, a reduction in acute care
hospitalization rate, improved patient satisfaction and implementation of a telehealth program. Cheryl is credentialed as a Home Health ICD-10 Coder and OASIS Specialist.
Cheryl is committed to having a voice for the home care industry and the people that are served. She feels it is important that leaders of home care ensure access to care
but also quality of care to those we serve all while staying compliant with rules and regulations of home health. With the implementation of the pilot program of

She has also served the last 2 years on the TAHCH board of directors in the Pediatric auspice and was recently elected President-elect. Kristen is a strong advocate for home health and hospice providers and the clients they serve.
About the Presenters: Brandis Wilmore is the founder and Administrator of Be Healthy at Home, a licensed and certified home health agency located in Buda, TX.
Her background is emergency medicine where she started her career as an EMT in Austin. She continued her education in the field of nursing and worked in many areas of the acute system which eventually lead her to home health.
She is a graduate of Texas State University in San Marcos with a focus in Business Administration. She currently sits on the Board for TAHCH and represents Small Agency. She also is the Chair for the Medicare/Hospice Committee.
Brandis is currently the disaster coordinator for her agency and works closely with her local CERT team to anticipate and act on her emergency preparedness plan for her clients and staff.
About the Presenters: Amy Ives has been a nurse for 23 years with the last 8 in the hospice arena. She is currently the Director for Peterson Hospice in Kerrville, Texas.

Program Description:Home Health agencies have a duty of care for the patients/clients they serve. A duty of care refers to the obligation to
take responsible care to avoid injury to a person whom, it can be reasonably foreseen, might be injured by an act or omission. Omission is often the area that puts agencies at greatest risk.
A clinician didn’t know how to meet a need, therefore, they overlook that it exist. In home health, the care you provide must be looked at through a multidisciplinary lens where multiple
complex issues will arise. As an Administrator, there will be times you are dependent on outside resources, and community partners, to solve a need. It is also important to understand
ethical issues associated with using outside resources, including ensuring that there are no “kickbacks” associated with using other community partners.

Program Description: The number of investigations and inspections by regulatory
surveyors has steadily increased over the past few years. In 2018, the Texas Health and Human Services Commission conducted
4,500 visits and contacts of home and community support services agencies. To help home health and hospice providers prepare
for such a visit, this program provides guidance on how to avoid the most frequent deficiencies cited by the state agency.
Examples of survey findings with the rationale for the finding of deficiencies are highlighted. Part 1 includes a review of the survey process and a review of data from the
Texas HHS Annual Report Regarding LTC Regulatory - published August 2019 - and then presents details on deficiencies for Home Health; Part 2 presents deficiencies for Hospice.

Program Description: Regulatory Compliance is of grave concern always, but especially during times of
heightened scrutiny by medical reviewers. CMS’ has stated concerns about the sources of clinical information for hospice certifications of terminal
illness (CTI) and other recent reminders from CMS emphasizes the importance of compliance with CTI and Medicare election statement requirements. These
items will be part of every medical review and there is a new focus on them during Medicare surveys. The comprehensive assessment of the patient is

Melinda’s current impact on the industry. She is currently serving on the NAHC/HHFMA Advisory Board as ex-officio and is Associate Director on the Home Care

Program Description:In this program the presenters review how agency outcomes affects star ratings under PDGM.
A case study is presented focusing on care of the patient with a specific primary diagnosis and
differences between caring for the patient under PPS versus PDGM. Key areas of focus will be managing
inappropriate LUPAs under PDGM, cost effective approaches for improving functional status of patients
under PDGM, and recommended metrics to track under PDGM.
Program Learning Outcome: Participants will actively participate in the learning activity and indicate an intent to change
their practice by implementing effective clinical approaches to contribute to improved patient outcomes under the Patient Driven Groupings Model.

About the Presenter: Christopher Attaya joined SHP in 2014 after spending 28 years in executive
and consulting positions within the Home Health and Hospice industry. In his role, he is responsible for product development and client relationships
to help organizations achieve increased operational and financial performance through the use of SHP’s industry leading analytics platform and benchmark
data. Prior to SHP, Chris was the CFO at the VNA of Boston and had worked at Partners Health Care at Home as CFO and CEO. He received a B.A. in Public
Health from Tufts University and an M.B.A. from the Graduate School of Management at Boston University concentrating in Health Care Finance.
About the Presenter: Sue Payne joined the Corridor team in December 2016 as part of the Transpirus
acquisition where she served as the Senior Vice President of Clinical Services. In her role, Sue is responsible for clinical quality and business review
to assure client satisfaction. She also serves as the Corporate Compliance Officer.
Sue has over 25 years in home care operations and strategic leadership. Prior to her role at Corridor, Sue was the Vice President of Home Health at
Advanced Home Care and at Life Care at Home, both of which are multi-state home care providers. Prior to that, she was the Administrative Director at
Bon Secours Home Care which is a multi-site hospital based home care agency. She has experience with hospital based, long term care and privately held home
care companies. Sue was the recipient of the 2011 Leadership Award from the Association for Home & Hospice Care of North Carolina. She has presented
nationally at Press Ganey, Visiting Nurses Association of America, American Congress of Healthcare Executives, and State Associations on topics such as
preventing avoidable readmissions, value based purchasing, staffing retention and early careerist mentoring. She currently serves as a VNAA board member
and is on the VNAA Governance Committee.
Sue has a B.S degree in Nursing from Radford University, a M.B.A from the University of Tennessee-Chattanooga and holds Certification in
Home Health/Hospice from the National Association of Home Care.
