Program Description: What’s now and what’s new? The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes.
Even though CMS is omitting the TNC from its programs and replacing it with the Discharge Function score, the TNC scores are still important through payment year 2026. Now is the time to ramp up for
the first performance year for Discharge Function, so learn what your plan should be for 2024. ACH and ED Use claims data currently comprise 35% of your Total Performance Scores. Even though your
ACH and ED Use scores are being jettisoned in favor of the new PPH, the ACH and ED Use will still impact your payment until 2026. New to VBP are the PPH and the Discharged to Community (claims-based)
quality measures, which together will comprise 35% and will require some adjustments in your Performance Improvement Programs. PPH will include hospitalizations (both ACH and LTCH) and observation stays.
The DTC measure will include readmissions for up to 31 days after discharge. Lisa discusses what actions can be taken now to improve those numbers.

Program Description: Home Health’s Value-Based Purchasing (VBP) Model is the latest of the Impact Act Volume to
Value reforms to affect how we develop and deliver care. By redefining HH quality in terms of Pay-for-Performance (P4P), VBP seeks to improve care efficiency.
As a result, traditional HH Care production and management processes fail to achieve VBP-based quality baselines and the related bonus payment.
When reviewing the HH VBP model, it quickly becomes apparent that the depth and specificity employed to track, define,
and reward qualified care extends well beyond the current level of management in most Home Health agencies. The complexity of VBP contrasts starkly
with both the Star Ratings and HHCAHPS models we know, and it is much more difficult to achieve success. Finally, HH VPB will deliver bonus payments
to the top 10% of HH Providers nationally, and few if any Providers could hope to qualify for the bonus without a major VBP-based quality initiative

Program Description: Part of your Total Performance Score (TPS) is based on OASIS outcomes. Currently it is based on two composite scores for self-care and
mobility (the TNC) along with improvement in dyspnea and oral medication management and discharged to community. This program takes a good look into the stand-alone OASIS items and describes how the TNC is calculated.
It also discusses the future changes to the TPS with the omission of the TNC from the TPS altogether. The TNC is currently 17.5% of the Total Performance Score of VBP, but CMS is implementing drastic changes by
omitting the TNC from its programs and replacing it with the Discharge Function score. Learn the impact of the changes and how your strategies should have a new focus in 2024.
