You are not alone! Hospice agencies across the nation are surprised that their comprehensive assessment composite, as well as some of the individual scores are lower than expected
per CMS data. Did you know that even if you answer positively to some of these process measures, CMS has "Validation questions" embedded in the HIS to ensure your answers represent
the measure, as CMS defines it. Literally, you may say "Yes, we screened the patient for pain"- but if any of the following questions - date, rating or type of standardized tool are
