of experience ranges from successfully holding positions as Agency Administrator, Clinical Nurse Manager, and corporate VP of Program
Development to leading organizations in successfully achieving growth and national accreditation. She has provided leadership in the
Home Care Association of Colorado as a liaison to the Department of Public Health survey department and served` as a member of the

Program Description: With Pressure Ulcer (PU) prevalence rates between 2.9% and 19.1% in home care,
it’s extremely important to manage wounds efficiently and effectively. In addition, with the coming changes of PDGM, home care providers will
be forced to expand into more skilled-nursing activities, such as wound care. Amy Cassata, VP Clinical at Swift Medical, has implemented hundreds
of wound care programs and shares her best practices over the years, including clinical best practices, technology recommendations, and

Program Description:With an ever-changing competitive market, providers need to consider
how to deliver services more efficiently. This program takes a look at real life case studies of how technology is being implemented
in home and community care to assist in the delivery of services. What technologies are being implemented to record and monitor changes
in an individual’s health? What is being implemented in regional and remote areas to ensure the wellbeing of seniors?
Machine learning is a type of artificial intelligence that is concentrated on developing algorithms that
analyze data and predict future events with tremendous accuracy. It is a technology that is well suited to identify hidden trends and
patterns in vast amounts of data, not visible to the human eye. Jonathan Vallée, CTO of AlayaCare, an end-to-end home healthcare software
solution, discusses the importance of collecting and analyzing data, provide real examples of feasible tech solutions organizations
could implement today, explore the insights gained from utilizing machine learning, and present the results of two studies on applying
machine learning to streams of incoming remote patient monitoring vitals for decision support.

Program Description: Home Health Providers in Illinois, North Carolina, Ohio, Florida or Texas are
gearing up for the implementation of the Review Choice Demonstration (RCD) in their states.
Following the Palmetto GBA RCD training presented 6/19/19, two distinguished Illinois providers - both board members of the Illinois HomeCare & Hospice Council - presented results
of preclaim review and review choice in Illinois - the first state to begin. They present data from Illinois, issues impacting Illinois providers, and each one presents their own agency's experiences - along with
practical tips and tested processes to help you be successful with RCD.

She has worked in the Home Care field for 23 years serving in various clinical and leadership roles. She resides in Bloomington, IL with her husband and three children.
Cheryl Adams RN, BSN, MBA with 40 years of clinical nursing and healthcare executive experience. Cheryl has been the administrator of At-Home
Health Care of Sparta Community Hospital since its inception in 1996. Cheryl is responsible for developing, implementing and sustaining the business plan for the hospital
based home health agency known as At-Home Health Care. The agency under her direction has a history of multiple deficiency free surveys, a reduction in acute care
hospitalization rate, improved patient satisfaction and implementation of a telehealth program. Cheryl is credentialed as a Home Health ICD-10 Coder and OASIS Specialist.
Cheryl is committed to having a voice for the home care industry and the people that are served. She feels it is important that leaders of home care ensure access to care
but also quality of care to those we serve all while staying compliant with rules and regulations of home health. With the implementation of the pilot program of
Pre-claim Review in IL, Cheryl has succeeded in achieving a 100% affirmation rate through the education and implementation of processes to ensure accurate,
complete documentation.

Irma Jacobson is a commercial litigator and administrative law attorney who
regularly practices in state, federal, and administrative law courts. She graduated from West Point in 2004 and served in Operation Iraqi Freedom
from 2005-2006. Irma has extensive experience representing clients in the health care industry and excels at providing legal solutions to complex
problems by tailoring strategies to fit each client’s specific need. She can assist with data privacy requests, both requesting disclosure and
defending against disclosure involving Health Insurance Portability and Accountability Act (HIPAA), Freedom of Information Act (FOIA), and the
Texas Public Information Act (TPIA).
Irma represents nursing home facilities, assisted living facilities, and home and community support services agencies (HCSSAs)
regarding compliance and licensure actions. She has been essential in securing favorable results for long-term care facilities while defending against
license revocations and appealing Medicare/Medicaid terminations and state and federal civil monetary penalties. She has extensive experience advising
long-term care facilities concerning audits by Medicaid Recovery Audit Contractors (RACs), Medicaid Integrity Contractors (MICs), Zone Program Integrity
Contractor (ZPIC), and other Medicare/Medicaid reimbursement matters. Her practice also includes representing benefit plans, providers, employers, claims

Program Description: Prior to Jan. 13, 2018, many of the things we “kinda” did okay, such as care coordination,
comprehensive assessment and plan of care management, became mandatory measures under the new Conditions of Participation. And agencies have struggled!
Surveyors continue to site quality of care, care coordination and care planning collectively as one of the top two deficiencies for agencies across the board.
By having an in depth understanding of Medicare’s definition of the four “C’s” along with how they build upon each other will lead to more streamlined processes,
develop effective care coordination practices and provide better, more patient-centered plans of care thereby helping to “survey proof” your agencies.

About the Presenter:
Apryl Swafford began her nursing career in 1993, obtaining a BSN from Jacksonville State University in Jacksonville, Alabama.
Her first few years were spent learning nursing, trial by fire style, in a 16 bed ICU unit. She transitioned into home health in late 1997 working PRN and weekends while learning her way and has
spent the majority of the years since working home health in some capacity. She has worked as a field clinician/case manager, clinical manager and leader of a cardiopulmonary specialty program.
During her tenure at the last two positions, she was responsible for chart audits, OASIS review and coding plus management of a team of nurses, therapist and home health aides.
Apryl is certified as an OASIS Specialist - Clinical (COS-C), and is a credential holder of the Homecare Coding Specialist (HCS-D) for both ICD-9 and ICD-10. During her free time, she enjoys
her family (there’s a lot to enjoy with 4 children and 9 grandchildren), working on the family’s small farm and running.
